Our family owned and operated nursing community offers skilled nursing care, rehabilitation service , long term care and assisted living under one roof. By providing multiple levels of care, we can best meet the changing needs of our senior population. Colonial Poplin , our 50 bed Medicare and Medicaid certified skilled nursing home and rehabilitation center, provides individualized services to meet diverse cognitive and physical abilities while Poplin Way Assisted Living gives residents the opportunity to live in their own place while receiving assistance when needed. At both Colonial Poplin and Poplin Way we believe in the person-centered approach to care, always treating our residents with dignity and respect, ensuring their total health needs are met, and they achieve their highest levels of functional independence.
